This is a tool for families. It is a checklist to make sure you have everything in order. You can also complete the information and leave the appropriate portion with your Medical Power of Attorney, or the Executor of your Estate. Remember to protect all information that is put in writing by making sure it is available only to the people who are trustworthy and will need the information to honor your wishes.
